Making a referral
For health and social care professionals placing someone with us.
Start a referral You will reach our team. We aim to respond the same working day, and we will tell you quickly if we cannot meet the person’s needs rather than leaving you waiting.
Access to our services is based on a robust assessment of need, carried out by our assessing team.
Prospective individuals may have an enduring mental health condition, a dual diagnosis, a physical disability or other complexities. An assessment allows us to identify suitability for support, as well as capacity to consent to care and tenancy management.
Referrals are accepted from both health and social care professionals.
How it works
Assessment
Two of our assessing team carry out an assessment of need, including capacity to consent to care and to a tenancy.
Decision and placement
We confirm whether we can meet the person’s needs, which home suits, and when a move could happen.
